All forms for clients can be filled out through an electronic portal before their first scheduled session.
How it Works:
- I will send you the link to your own client portal after we complete a phone consultation or you schedule an appointment for a session. The client portal is used to submit paperwork, schedule sessions, and pay for sessions.
- The initial paperwork will ask you to provide some information to me about your treatment needs and goals and other related information.
- The therapy paperwork also includes forms that explain information about treatment, confidentiality and your HIPPA rights, information about telehealth therapy sessions, and authorization for Peaceful Shore Therapy, PLLC to bill your credit card for sessions.
- Finally there is a release of information form that allows us to provide or receive information with other mental health professions, health care providers, or other agencies or individuals you specify.
